To Register For Day Camp

Before registering your child with the Day camp, we ask you to read the letter to the parents as well as the code of conduct and to return it signed with the necessary forms.

To register your child with the Day camp, you must fill out the REGISTRATION FORM. Please fax or send the registration form to the Township of Alfred and Plantagenet office at  205 Old Highway 17, Plantagenet, ON  K0B 1L0, or Fax: (613) 673-4812, so we can plan the Day camp activities.

If your child has a medical condition, you must fill out the MEDICAL FORM, as well as the MEDICATION FORM, if it your child need to takes a medication or the ALLERGIES FORM if your child must have an injection for an allergy.

We ask you to fill out the AUTHORISATION FORM FOR THE WEB SITE, because we would like to be able to use these photographs for promotional purposes.

General Information:

Dates: June 25th to August 17th, 2007.
The camp will be held at the Plantagenet Park at 550 Albert Street, Plantagenet.
The hours are from 9:00 a.m. to 4:00 p.m., Monday to Friday.
Babysitting/supervision services will be available from 7:00 to 9:00 a.m. and from 4:00 to 6:00 p.m., from Monday to Friday. Additional fees will be applicable for these services.
Parents must assume transportation arrangements for the arrival and departure of their child(ren).
A bus shuttle is scheduled from Alfred only if there is sufficient registration for the service ( leaving Larocque Park at 8:45).
Children will be grouped according to their age.
An outing or special activity is scheduled every week; the cost is included in the weekly fee.

Registration:

Monday, June 11th, 2007, Larocque Park in Alfred, from 6:00 to 8:00 p.m., at 555 St-Philippe Street.
Tuesday, June 12th, 2007,  in Plantagenet Scouts Local, from 6:00 to 8:00 p.m., at 550 Albert Street.
Additional information will be given to you at the registration.

Cost:

Per week: 1^st child - $50; 2^nd child - $30; 3^rd child - $15.
Per day: $20.00 per child
An outing only:  $30.00 per child
Child care services:  $3.00/hour per family
